Family Support can help fund a variety of services

The Family Support Services Program is a program funded by the State of Ohio and facilitated by the Lucas County Board of Developmental Disabilities.  This program is designed to provide reimbursement in a variety of areas, counseling/training/education, special diets, adaptive equipment, home modifications, and respite care.  All requests need to relate to the needs of the individual’s disability.  Individuals need to reside within their family home to qualify to access the fund. Read our welcome letter.

If you have any questions regarding the use of the Family Support Services Program contact the Children’s Dept. at 419-381-6139.

Below you will find the necessary forms to request use of the funds.

You may return completed forms by US Mail or in person.  You will find the return contact information at the bottom of each form.  Please do not send forms as pictures from your cell phone or tablets.
